The following language reflects the order acknowledgements present in the application that require acceptance from both an order submitter and a clinician.
Clinician Acknowledgements
A clinician acknowledgment is required for Clinical projects and is presented at clinician registration. The acknowledgment serves as an affirmation for all orders within the project. Below is the language:
- I attest that I am authorized under applicable law to allow for the order of the tests in this project.
- I attest that I have obtained Informed Consent from the patient(s) (or their legally authorized representative) and that I obtained any other consents from the patient as required by applicable state law and/or regulations in order to perform a genetic test on a specimen. I will maintain all written consent forms as part of the patient file and make them available upon reasonable request. The results of this test may be used in the medical management of the patient and/or genetic counseling of the patient and their family.
- I certify that all specimens have been collected, handled, and processed in a manner consistent with BCL instructions. If the sample has been extracted, I attest that the genomic material/nucleic acid samples being submitted were extracted in a CLIA-certified laboratory or a laboratory meeting equivalent quality system requirements.
Order Acknowledgment
An order acknowledgment is required in clinical and clinical reference lab projects at order submission. Below is the language:
- I attest that I am authorized under applicable law to order this test.
- I attest that I have obtained Informed Consent from the patient (or their legally authorized representative), and that I obtained any other consents from the patient as required by applicable state law and/or regulations under the laws of my state and/or regulations in order to perform a genetic test on a specimen. I will maintain all written consent forms as part of the patient file and make them available upon reasonable request. The results of this test may be used in the medical management of the patient and/or genetic counseling of the patient and their family.
