Overview
This article will walk you through a basic step-by-step guide on how to place an order in the ordering application. Not all steps are relevant for each test type and will be noted accordingly.
Ordering Steps

Test Information – this step is used to select the Project, Clinician, and Tests to order.
- Test ordering options such as CSV upload and DTP Kit requests are also selected in this step.
- Patient Demographics and Sample Information – this step is used to enter the patient and sample details for the order.

HPO/Phenotype Input – this step is used to enter HPO/Phenotype terms that may apply to the patients within the order.
- Note: This step will only be displayed if required for the test being ordered (e.g. for Diagnostic Genome tests); otherwise, it will not be visible.

Clinical Information – this step is used to enter the clinical information and patient history.
- Note: This step will only be displayed if required for the test being ordered (e.g. for Diagnostic Genome tests); otherwise, it will not be visible.
- Review and Submit – this step is used to review the key information for all patients and samples included within the order, to verify the attestations, and to submit the order.
Step-by-Step Guide

Begin an Order
To begin an order, start on the Orders Dashboard and click the +Add Order button.
Next, select your project from the Project Name dropdown list. This will enable an active set of clinicians and test types available.
Then, select your Clinician (if applicable) and Test Type. Add on any additional optional screening panel tests, if applicable, and specify the family members.
If using Direct to Patient kits or Bulk Upload, ensure the appropriate checkbox is selected before proceeding.
Click Save and Continue to move forward in the ordering process.

Enter Patient Demographics and Sample Information
Enter required patient and sample information. All required fields have a red asterisk next to the field name.
To add multiple patients to the order, select +Add Patient at the bottom of the screen.
Once all patient and sample information has been entered, continue by either selecting Save and Continue or resolving errors on the screen.

Enter HPO and Phenotype Information
This step is only applicable when ordering a Diagnostic Genome test.
Search and select all phenotypic features and symptoms present in the proband and any family members included In the order.
Once all phenotypic information has been entered, continue by either selecting Save and Continue or resolving errors on the screen.

Clinical Information
This step is only applicable when ordering a Diagnostic Genome test.
Enter required information, including Indication for Testing and Family History questions. Other test and/or project types may require other and/or additional fields.
Information may be entered as free-text or as files within the Upload Documents section of the page.
Once all clinical information has been entered, continue by either selecting Save and Continue or resolving errors on the screen.

Review and Submit Order
Review all patient, sample, phenotypic, and clinical information before submitting the order.
Review the order attestations and select the red checkbox to confirm your agreement with the statements. Once attestations are confirmed, you may submit the order.
