As a new user to the system, you must register your account by creating a password and entering your personal profile information into the fields as described in the table below:
Note: All fields with an asterisk ( * ) are required.

| Clinical Institution * | Name of the Clinical Institution with which you are affiliated. |

| State * | Clinician Institution's state (select from dropdown list) |
| Postal Code * | Clinician Institution's zip code in the US, or other postal code if outside the US |

| NPI Number * | Your provider NPI number – which must be 10 numeric digits. |
| Email ID | Your email id – auto-filled based on your registration email. This field may not be modified. |
| Roles by Project | The role(s) and project(s) to which you’ve been invited. This info may not be modified during registration. |
| Attestations * | Clinicians have specific attestations that must be agreed to for each individual project to which the Clinician is associated. The screenshots below will provide more detail. |

Attestations
Clinicians have specific attestations that must be agreed to for each individual project to which the Clinician is associated.
As an existing user with a Clinician role, when you are invited to any additional projects, you will skip the upper portion of the Registration page, but instead will only have to complete the attestations for the given project.
After entering all required information, you may click on the Register button to continue to the ordering application.
